Gentamicin - Pediatric Dosing

Preferred in the setting of normal renal function

Serum Level: PRE level 0 to 30 minutes before dose

Initial Level: Day 3

Targets:

  • PRE (Trough) levels: Less than 0.3 mg/L

  • POST (Peak) levels: Not recommended. Consult Pharmacy if required

Daily Dose: 7.5 mg/kg/day

Maximum Daily Dose: 360 mg/dose before levels

Dosing Frequency: IV divided q24h

Select when dose will be adjusted based on analysis of drug levels

Serum Levels:

  • PRE (Trough): 0 to 30 minutes before dose

  • ┬áPOST (Peak): 30 minutes after end of 30 minute infusion

Initial Levels: Dose 3

Targets:

  • PRE (Trough) levels: Less than 2 mg/L

  • POST (Peak) levels: 5 to 10 mg/L

Daily Dose: 7.5 mg/kg/day

Maximum Daily Dose: 120 mg/dose before levels

Dosing Frequency: IV divided q8h

Serum Level: PRE level only, 0 to 30 minutes before dose

Initial Level: Dose 3

  • POST (Peak) levels: Not recommended. Consult Pharmacy if required

Daily Dose: 3 mg/kg/day

Maximum Daily Dose: 80 mg/dose before levels for q8h dosing and 240mg/dose before levels for q24h dosing

Dosing Frequency: IV divided q8h or q24h*

*In adults, single daily dose is preferred for endocarditis though TID dosing is an alternative. Data for once-daily dosing of aminoglycosides for synergy in children do not exist.
