Vancomycin is inferior to nafcillin or cefazolin for the treatment of MSSA infections.
Infectious diseases consultation is strongly advised for all patients with S. aureus bloodstream infections to ensure optimal therapy.
Treatment should be parenteral. Oral antibiotics are not appropraite for S. aureus bloodstream infections.
Infected line must be removed, salvage should not be attempted.
Nafcillin 2 gm IV every 4 hr
Cefazolin 2 g IV every 8 hr
(Non-anaphylactic PCN allergy)
14 days is the minimum duration of therapy for S. aureus bloodstream infections, and should only be considered if endocarditis or other metastatic infections have been ruled out.
