Ethambutol - Pediatric Dosing


Dosing based upon lean body weight

Infants, Children and Adolescents Less Than 15 years and Less Than 40kg

15 to 25 mg/kg/dose-PO Q24H

Infants, Children and Adolescents Less Than 15 Years and Greater Than 40kg

Refer to adult dosing

Adolescents Greater Than 15 years

Refer to adult dosing


Dosing adjustment not clearly established in pediatrics. Ethambutol is primarily renally eliminated and based upon available pharmacokinetic/pharmacodynamic data in adults, dose adjustment is warranted. The following recommendations are based upon the ADULT ATS/CDC/IDSA recommendations (2016) and only serve as a guideline

CrCl < 30 mL/min

20-25 mg/kg/dose three times weekly. Do not administer daily dosing.


20 to 25 mg/kg/dose three times weekly. Given after dialysis on dialysis days.

Peritoneal Dialysis

Administer every 48 hours

Continuous Renal Replacement Therapy (CRRT)

Administer dose every 24H to 36H
