Loading dose, all neonates

A loading dose of 25 mg/kg/dose IV/PO x 1 dose may be considered in select neonates with suspected or confirmed systemic Candidiasis with any of the following risk factors: 

  • Suspected CNS involvement
  • Severe infection
  • Patients on ECMO

Documented Candidiasis or Clinically Suspected Invasive Candidal Infection, corrected gestational age less than or equal to 29 weeks

Post natal age less than or equal to 14 days

12 mg/kg/dose IV/PO q48h

Post natal age greater than 14 days

12 mg/kg/dose IV/PO q24h

Documented Candidiasis or Clinically Suspected Invasive Candidal Infection, corrected gestational age greater than or equal to 30 weeks

Post natal age less than or equal to 7 days

12 mg/kg/dose IV/PO q48h

Post natal age greater than 7 days

12 mg/kg/dose IV/PO q24h

Prophylaxis for Prevention of Invasive Candidiasis

Consider prophylaxis only in extremely low birth weight infants (birthweight less than 1 kg)

Starting on day 3 of life: 3 mg/kg/dose IV/PO q72h and continue until 6 weeks of age or until IV access no longer required.

Thrush, gestational age less than or equal to 29 weeks

Post natal age less than or equal to 14 days

6 mg/kg/dose IV/PO on day 1, followed by 3 to 6 mg/kg/dose IV/PO q72h ; change to q24h once beyond the first 2 weeks of life

Post natal age greater than 14 days

6 mg/kg/dose IV/PO on day 1, followed by 3 to 6 mg/kg/dose IV/PO q24h

Thrush, gestational age greater than or equal to 30 weeks

6 mg/kg/dose IV/PO on day 1, followed by 3 to 6 mg/kg/dose IV/PO q24h
