Duration of therapy

Uncomplicated ( Afebrile with significant clinical improvement after 3-7 days of appropriate IV therapy):
3-4 weeks, based upon clinical response. Consider switch to oral cephalexin to complete therapy if clinically improved and inflammatory markers have started to normalize . C-reactive protein testing is recommended to monitor response to therapy and should be normal before stopping therapy.

If imaging demonstrates progression of bone disease, additional evaluation for lesions that may require surgical treatment (eg abscess, sinus track, sequestra) should be undertaken, and IV antibiotic therapy continued

Requires longer duration of therapy for a total duration 3 months or longer depending on response
