Loading dose 300mg IV infusion q12h on day 1
Followed by 300mg IV infusion q24h starting on day 2

  • No adjustment needed for renal impairmentLoading dose of 300mg PO q12h on day 1
    Followed by 300mg PO q24h starting on day 2- Prophylaxis: 200mg PO three times daily

  • Treatment of invasive fungal infections: 400mg PO q12h OR 200mg four times daily for patients unable to tolerate a meal or nutritional supplement

IV: Accumulation and resultant toxicity of the diluent can occur if CrCl <50 mL/min
