Notes

For Procedures with P and T approved order sets, prophylaxis as outlined on these order sets is also acceptable.

NOTE: Re-dosing interval refers to the frequency at which additional drug should be given intra-operatively during extended procedures. This is NOT the same as the usual dosage for the drug.

(i.e. cefazolin re-dosing interval during procedure is q4h, but normal dosage interval for cefazolin outside the O.R is q8h)
