Non-invasive gram-positive infections (e.g., skin and soft tissue infections):
4-6 mg/kg IV q24h
Bacteremia and invasive infections:
staphylococci (including methicillin-resistant) and enterococci: 6-10 mg/kg IV q24h
vancomycin-resistant enterococci (VRE): 8-10 mg/kg IV q24h; 12 mg/kg IV q24h may be required for highly resistant strains
Endocarditis:
Methicillin-resistant staphylococci: 8-10 mg/kg IV q24h
Resistant enterococci (including VRE): 10-12 mg/kg IV q24h
